Our requirements are:

The fully completed application with original signatures form the applicant, co-applicant and academy. The terms and conditions must also be initialed by applicant and co-applicant.

A signed sales slip for services with the dollar amount being financed.

A Legible copy of the applicant and co-applicant's driver license and or military identification card.

A legible copy of the back and front of a current credit card form the applicant and co applicant. (If they do not have a credit, the a copy of voided check and a copy of their social security card will suffice)

A legible copy of voided check form checking account of applicant and co applicant.

Copy of a recent Utility bill, preferably a phone bill, from applicant and co applicant.

Copies of past two pay stubs from applicant and co applicant. If self employed we will accept copies of the last 3 monthly bank statements, showing deposits, as proof of income.
