     We want to help you get on the road to success! Remember, you must be over 21 years old to participate in Batavia's Driving Academy except for a standard Class B license. Please complete the information request below and our Academy Staff will contact you as soon as possible.

Pre- Enrollment Information Request

This does not enroll you with the Academy. You have to appear in person
with a copy of your motor vehicle record to enroll.Please fill out this form completely.  Your information request will be reviewed and responded to as soon as possible. If you require prompt attention, please call us at our office.  All information provided will never be shared or used for any purpose outside of the Academy operations.
